Canon 2 of the First Council of Orange (Gaul, AD 441) forbids ministers of the altar from baptizing without a chrism.
[Titulum ex cod. M] II. De baptizatus esse chrismandus.
[Titulum ex cod. A, R] II. Si baptizatus quis, necessitate faciente, chrismatus non fuerit.
[Titulum ex Collectione Hispana] II. Vt nullus minister sine chrismate proficiscatur.
 
Nullum ministrorum, qui baptizandi recipit officium, sine chrismate usquam debere progredi, quia inter nos placuit semel chrismari. De eo autem qui in baptismate, quacumque necessitate faciente, non chrismatus fuerit, in confirmatione sacerdos commonebitur. Nam inter quoslibet chrismatis ipsius nonnisi una benedictio est, non ut praeiudicans quidquam, sed ut non necessaria habeatur repetita chrismatio.

No minister who was granted the right to baptize should proceed in whatever situation without a chrism, because we decided among us that one should be anointed with chrism only once. If a person was not anointed with chrism during baptism by whatever reason, the priest should be reminded of that fact during confirmation. For in different anointments of chrism there is only one blessing, not that there is something harmful in repeating anointments, but one should not think that it is necessary to repeat anointment with chrism.

This canon is repeated in somewhat changed form in the so-called Second Council of Arles as canon 27 (26) (see Munier 1963: 120; e.g. [549]).
